ELIGIBILITY
Because SDAC is a state-funded home health program, there are a few eligibility requirements, learn about them below.
The consumer (person needing care) is actively enrolled in AHCCCS
The consumer (person needing care) is a AZ resident & 18+
The consumer (person needing care) needs assistance with daily activities
