We want to recognize their efforts by highlighting a caregiver every month for their hard work, kindness, and positive impact on the lives of those they care for.

Selection Criteria and Announcement
The caregiver being recognized will receive*

An award certification for being the 'Caregiver of the Month'

$50 gift card

An exciting package as a token of appreciation from BURD Home Health
All nominations will be collected, and a winner will be randomly selected from the list. The chosen caregiver will receive an email with more details.
*Items listed above may be adjusted or substituted at our discretion.
Nomination Criteria to be Caregiver of the Month
To be considered for the nomination pool, we verify nominations based on the following criteria:

The caregiver must be nominated by the person they are caring for or BURD Home Health staff.

The caregiver must have at least 90% EVV compliance.
Nominations are open for the entire calendar month (from the 1st to the last day of the month), and the ‘Caregiver of the Month’ will be announced in the first week of the following month.
Note: Each caregiver can be nominated 1 time per month.
