
Transition to PPL With BURD Home Health
BURD Home Health is an approved CDPAP facilitator that can help with the transition process and beyond.
It’s Not Too Late to Transition
New York State required all CDPAP participants to transition to Public Partnerships LLC (PPL), the new statewide fiscal intermediary, by August 1, 2025. If you haven’t completed your transition, it’s critical to act immediately to avoid any disruption in services.

Choose BURD Home Health as Your Facilitator
If you are a consumer and have not started the registration process, you can select BURD Home Health as your facilitator when you create your PPL@Home account.
If you have already started registration or are assigned to another facilitator follow these instructions to switch to BURD Home Health as your facilitator.
